Resumo
Introdução: A infecção pelo vírus chikungunya pode evoluir, em parte dos pacientes, para manifestações osteoarticulares persistentes, com dor crônica, rigidez, limitação funcional e importante impacto na qualidade de vida. Embora a fase aguda da doença seja bem reconhecida clinicamente, os mecanismos envolvidos na progressão para a artrite crônica ainda não estão completamente esclarecidos. Objetivo: Analisar os determinantes biológicos, imunológicos e clínicos relacionados à progressão da infecção aguda por chikungunya para a artrite crônica persistente, com foco nos mecanismos fisiopatológicos, nos fatores de risco e nas repercussões funcionais da doença. Metodologia: Trata-se de uma revisão integrativa da literatura, realizada nas bases PubMed/MEDLINE, BVS/LILACS e SciELO. Foram utilizados descritores controlados e termos livres combinados por operadores booleanos, com recorte voltado às manifestações articulares crônicas associadas ao vírus chikungunya. Após aplicação dos critérios de elegibilidade, 37 estudos compuseram o corpus final da revisão. Resultados e Discussão: Os estudos analisados mostram que a cronificação da chikungunya tem caráter multifatorial e envolve a interação entre persistência antigênica, ativação imune sustentada, produção prolongada de citocinas pró-inflamatórias, alterações na regulação osteoclastogênica, predisposição genética e mecanismos de sensibilização central da dor. Também foram identificados fatores clínicos associados à pior evolução, como dor intensa na fase aguda, poliartralgia, acometimento de múltiplas articulações e presença de comorbidades. No entanto, os achados da literatura são heterogêneos, especialmente em relação ao valor preditivo dos marcadores laboratoriais, o que reforça a complexidade fisiopatológica da artrite pós-chikungunya. Conclusão: A artrite crônica pós-chikungunya deve ser compreendida como uma condição heterogênea, resultante da interação entre fatores inflamatórios, imunológicos, estruturais e neurofisiológicos. O reconhecimento dessa complexidade pode contribuir para uma abordagem clínica mais precisa, além de reforçar a necessidade de estudos prospectivos e terapêuticos mais robustos.
