Fluid administration in critically ill patients: the importance of fluid balance in preventing generalized edema

Abstract

This study is an integrative literature review aimed at analyzing the relationship between fluid administration, fluid balance, and the prevention of generalized edema in critically ill patients admitted to intensive care units. The review followed the methodological steps proposed by Whittemore and Knafl, including the formulation of the research question, literature search, study selection, critical appraisal, and synthesis of findings. The search was conducted in the LILACS, SciELO, BDENF, PubMed, and MEDLINE databases using Health Sciences Descriptors (DeCS) and Medical Subject Headings (MeSH), considering publications from 2015 to 2025 in Portuguese and English. The analyzed studies demonstrated a consistent association between positive fluid balance, fluid overload, increased incidence of generalized edema, organ dysfunction, and worse clinical outcomes in critically ill patients. These findings highlight the importance of rigorous fluid balance monitoring as a fundamental tool for guiding fluid therapy and preventing complications related to excessive fluid administration.
