Osseous metaplasia: mandibular lesion adherent to the muscular fascia – diagnosis and surgical management

Abstract

Bone metaplasia is a rare pathological condition characterized by the formation of bone tissue in atypical anatomical sites, such as muscles and other soft tissues. When it occurs in the mandible and is adherent to the muscular fascia, it represents a relevant diagnostic and therapeutic challenge due to its clinical and radiographic similarity to neoplastic and inflammatory lesions. This study aims to report a clinical case of mandibular bone metaplasia adherent to the muscular fascia, highlighting the differential diagnosis, the surgical approach adopted, and the clinical outcomes. This is a prospective case report with a qualitative and descriptive design, conducted at the São Lucas Dental Center in Porto Velho, Rondônia, Brazil, in accordance with the ethical principles established by National Health Council Resolution No. 466/2012. Data were obtained through direct clinical observation, imaging examinations, photographic documentation, and histopathological analysis. The primary outcome was the complete surgical removal of the lesion, with preservation of adjacent anatomical structures and relief of symptoms. As a secondary outcome, satisfactory mandibular functional recovery and absence of recurrence during the follow-up period were observed. Data analysis was based on current scientific literature, allowing a critical discussion of the clinical implications of bone metaplasia, its possible etiological factors, the need for complementary diagnostic examinations, and the most appropriate therapeutic strategies. This study contributes to expanding knowledge about uncommon manifestations of bone metaplasia and assists dental practitioners in making more accurate clinical decisions in similar cases.
