Ictero-Febrile Syndrome Associated with Bicytopenia – Infectious Diseases vs. Hematologic Diseases: A Case Report

Abstract

This report describes the case of an elderly patient living in a rural area admitted with jaundice-febrile syndrome, skin lesions, gastrointestinal symptoms, and bicytopenia with febrile neutropenia. Initial investigation prioritized systemic infectious etiologies, without identifying a causative agent. Flow cytometry showed a population of T lymphocytes with a profile suggestive of clonality, raising the hypothesis of a lymphoproliferative disorder. However, bone marrow biopsy demonstrated normocellularity and mild T lymphocytosis of undetermined significance, without diagnostic criteria for lymphoma, and colonic biopsy did not show gastrointestinal involvement. The patient improved clinically and partially recovered from the cytopenias, being discharged from the hospital without a definitive diagnosis, with outpatient follow-up. This case highlights the challenges in differentiating between systemic infectious processes and hematological diseases, emphasizing the importance of longitudinal follow-up and hospital clinical investigation.
